Baylor College of Medicine Children’s Foundation -Uganda (Baylor-Uganda) is an indigenous not-for-profit child health and development organisation affiliated to the Baylor College of Medicine International Paediatric AIDS Initiative (BIPAI), a Network of paediatric HIV/AIDS care and treatment Children’s Clinical Centres of Excellence and international program offices in 11 countries across Africa and Eastern Europe.

Job Summary:  The Social worker is responsible for ensuring retention of clients into care, strengthen linkages and referrals between communities and Baylor Uganda PIDC clinic. They are responsible to support problematic clients through working with health workers, community volunteers, peers and other community structures to attain the global target of 95-95-95.

Key Duties and Responsibilities:  

  • Implement and monitor the community health linkage work plan through adoption of the CQI approach and DSDM activities on targeted HTS and CCLAD models of care.
  • Create awareness, identify and enroll eligible clients for the community based DSDM models.
  • Identify and address barriers to ART adherence and appointment keeping among clients at the facility and in the community. Contact and attend to clients who miss appointments and propose mitigating measures in order to improve client retention in care as per the clinic SOPs.
  • Ensure the routine update of client phone contacts and home addresses at every clinic visit and document in the EMRx in order to enable patient tracking and foster good retention.
  • Update the community/ facility referrals and linkages register and ensure no empty data field. Call or track clients to ensure that they received the services for which they were referred for.
  • Build capacity of community structures including the COE based PHA network to establish/strengthen linkages and referral systems/processes as well as provision of HTS, OVC and GBV services through trainings and mentorships.
  • Regularly track department indicators in line with project objectives and work plan.
  • In liaison with the community stake holders’, maintain a vibrant functional network of PLHIV, community health workers, family support groups and other community stakeholders.
  • Develop and implement strategies for demand creation and communitymobilization including the involving of the community to curb barriers to health seeking behaviour and factors hindering behaviour change plus promotion of male involvement in the interventions in line with project objectives for MCH and HIV/AIDS.

Qualifications, Skills and Experience:

  • The applicant for the Baylor-Uganda Social Worker job should hold a diploma in humanities studies
  • Three years’ experience and knowledge in HIV/AIDS prevention, care and treatment
  • Good interpersonal and communication skills
  • Ability to solve psychosocial problem
  • Good counseling skills
  • Good motorcycle riding skills
  • Good computer skills
  • Team oriented with high integrity
