About US:
The Ministry of Health (MOH) has received a grant from the Center for Diseases Control and Prevention (CDC) Cooperative Agreement (CoAg), a strategic mechanism of the United States Government (USG) to implement activities in line with its mandate in support of the Public health response to HIV/AIDS and other health systems. CDC will be supporting a pool of Human Resources that are expected to lead the implementation of the grant. The staff to be recruited under this grant will be contracted by the MOH. The contracts are for one (1) year renewable subject to availability of funds and satisfactory performance of the staff
Job Summary: Supporting the hospital management to provide psychosocial services i.e. HIV case management to patients and their families receiving services in the HIV/ART Clinic
Key Duties and Responsibilities:
- Identifying and enrolling clients eligible for HIV case management using HCM criteria.
- Providing timely tracking and following up of patients under HIV case management.
- Reviewing progress and graduate eligible patients from HCM to appropriate DSDM.
- Providing psychosocial support to all continuing clients to adhere to medications, stay in care, virally suppressed and routinely screen for GBV.
- Conducting and coordinating health education sessions at ART Clinic.
- Timely documenting and updating of patient information in appropriate registers, HIV care cards under HIV case management.
- Participating in weekly/monthly/quarterly data reviews meeting and giving feedback to
- Clinical staff on patient progress under HCM.
- Providing monthly technical reports to health facility and cluster technical supervisors.
Key outputs
- Eligible clients enrolled in HCMM and followed up.
- Patients reviewed with adherence plans, graduated from HCMM and enrolled to appropriate DSDM.
- Adherence sessions documented in the patient’s HIV care card.
- Screening for GBV ,psychosocial support given and referrals.

Qualifications, Skills and Experience:
- The ideal applicant Bachelor Degree or an equivalent of Social Work and Social Administration or Adult and Community Education or Community Psychology or Development studies.
- One (1) year of experience in implementing community targeted HIV and MNH interventions
